Operation Serpent is a free campaign that adds new maps, enemies and a story, packed into serpent1.pk3 (28 MB) with a credits file. Release 0.991 is the last build of the mod. Load the pk3 with a source port that supports it.

The launcher installs this into mods/wolfenstein-operation-serpent/ for Wolfenstein 3D via ECWolf. If the base game is missing, it will install that first.
